健診の対象者には、通知を発送しています。生まれ月により申し込みできる日程が決まっています。We have sent a letter to those eligible for a checkup. The dates you can apply for are determined by the month of your child's birthday.

＜内容＞< What the checkup involves >
　尿検査、保健相談、歯科健診、身体計測（体重・身長）、内科診察、
育児相談（保健師・管理栄養士・視能訓練士・心理相談員による個別相談）
Urine test, health consultation, dental checkup, physical measurements (weight and height), internal exam, consultation on childcare (child development, food, eyes, etc.)

＜持ち物＞
・母子健康手帳（保護者の記録を事前にご記入ください）
Mother and Child Health Handbook (Please fill in the records for the parents/guardians at home)
・バスタオル等の敷くもの（衣類着脱時や歯科健診時使用）Large towel (used for changing clothes and for the dental checkup)
・目と耳のアンケートEyes and ears questionnaire
・健診アンケートHealth questionnaire
・尿検査(ご自宅で当日採尿し、お持ちください）Urine test (Please take a urine sample at home on the day of the checkup and bring it with you）

＜ご注意＞
・健診の対象者には、通知を発送しています。必ずお読みください。We have sent a letter to those eligible for a checkup. Please be sure to read it.
・完全予約制です。Please be sure to make an appointment.
・感染予防対策を強化しています。以下ご協力ください。
In order to prevent infections, please abide by the following rules.
①お子様一人につき保護者一人の同伴でお願いします。（健診対象外のきょうだいの同伴不可）
Only one parent or guardian should accompany each child. (Other siblings are not allowed to come)
②自宅で検温し、体調を確認してください
Please take your child’s temperature at home and check their physical condition.
③お子様やご家族（保護者やきょうだい）に発熱（37.5℃以上）、咳などの体調不良の方がいる場合は当日の受診を控え、無理せずに別の日程の受診を検討してください。
If the child or a family member (parent/guardian or sibling) has a fever (37.5 degrees Celsius or higher), cough, or other health problems, please cancel the appointment. Please come another day instead.
