・医療機関での健診結果「1歳6か月児健康診査受診票（丙）結果通知書（白色）」Results from the hospital checkup “18-month child health checkup sheet (C) results notification form (white)”
・バスタオルA large towel
・「1歳6か月歯科健診・育児相談」アンケート　“18-month old dental checkup / consultations on childcare” questionnaire
・「健やか親子２１」アンケート

＜ご注意＞
・生まれ月により利用できる日程が決まっています。通知を発送していますので、必ずお読みください。The available dates are determined by the month of your child's birthday. We have sent you a letter. Please be sure to read it.
・完全予約制ですPlease be sure to make an appointment.
・感染予防対策を強化しています。以下ご協力ください。In order to prevent infections, please abide by the following rules.
①お子様一人につき保護者一人の同伴でお願いいたします。（健診対象外のきょうだいの同伴不可） Only one parent or guardian should accompany each child. (Other siblings are not allowed to come)
②自宅で検温し、体調を確認してください。Please take your child’s temperature at home and check their physical condition.
③お子様やご家族（保護者やきょうだい）に発熱（37.5℃以上）、咳などの体調不良の方がいる場合は利用を控えてください。If the child or a family member (parent/guardian or sibling) has a fever (37.5 degrees Celsius or higher), cough, or other health problems, please cancel the appointment.
